On October 31, 2016, Public Health Management Corporation (PHMC) was notified that a potential security breach may have occurred involving information used by Turning Points for Children. A breach may have resulted from the theft of a workplace laptop, which was stolen from an employee’s locked car. Information from various case notes includes dates of birth, social security numbers and home addresses. Most of the individuals who may have had their personal identity breached have been contacted. Numerous and concerted attempts to contact the remaining individuals have been made, including site visits to last known addresses and the delivery of registered letters. Unfortunately, those attempts were not successful. PHMC has enforced policies to prevent unauthorized access to member information. If you receive services from Turning Points for Children/CUA9 and are concerned that your personal information may have been compromised, please contact 1.844.605.8490. It is recommended that you closely monitor your financial accounts. If you see fraudulent activity, you should contact the Federal Trade Commission immediately at 1-877-ID-THEFT (1-888-438-8228), or at www.ftccomplaintassistant.gov
