On February 24, 2017, Public Health Management Corporation (PHMC) was notified that a potential HIPAA security breach may have occurred involving information used by Turning Points for Children/CUA3. A breach may have resulted from the theft of a workplace laptop, which was stolen from an employee’s locked car. Information from various case notes includes medical records and court and school documents. PHMC has enforced policies to prevent unauthorized access to member information. If you receive services from Turning Points for Children/CUA3 and are concerned that your personal information may have been compromised, please contact 1.844.605.8490. It is recommended that you closely monitor your financial accounts. If you see fraudulent activity, you should contact the Federal Trade Commission immediately at 1-877-ID-THEFT (1-888-438-8228), or at www.ftccomplaintassistant.gov
